Shift Accountability and NYS-Compliant Care Tracking for an Assisted Living Facility
The Challenge
Care quality depended on staff reliably reading and recording shift transfer notes, but nothing held anyone accountable for doing it. On top of that, New York State requires detailed, auditable records across resident onboarding, discharge, evaluations, and care plans, and gaps in that documentation expose an assisted living facility to steep NYS fines.
The Solution
Built a two-sided shift accountability system: a kiosk where staff read the latest transfer notes at the start of a shift, and a web portal where admins and nurses add them. Paired it with a NYS-compliant Assisted Living Facility dashboard that tracks resident onboarding and discharge, resident evaluations, medical evaluations, Individualized Service Plans, notes, diet logs, and weight logs in one place.
The Result
Staff are accountable for reading and acting on shift notes, and residents get more consistent care because information no longer falls through the cracks between shifts. The dashboard keeps the required NYS records complete and auditable, protecting the facility from the heavy fines that come with documentation gaps.
